ENROLLMENT
INFORMATION

These enrollment forms are
designed to be filled out electronically and printed
at home. You will not be able to save the forms once
you start; they must be printed or your information
will be lost.
After completing and printing the on-line enrollment
forms please bring them to the school along with
your student's certified birth certificate, shot
records, residency verification, and two utility
bills.
You may also visit the school to fill
out your child's forms.
Please
download and read the district's Network Agreement
before completing the enrollment packet for grades
1-5. We did not include this in the forms in order
to cut down on the number of pages printed.
