After printing and completing the enrollment forms please bring them to the school along with your student's certified birth certificate, shot records, residency verification, and two utility bills.

You may also visit the school to fill
out your child's forms.

1. Certified birth certificate

2. Residency Verification-closing settlement OR rental agreement

3. Two utility bills-gas, electric, phone, or water

4. Immunization records

5. Completed enrollment forms
